Job summary

Affinity Health Management is seeking a Clinical Manager to oversee our Pierce County team, with the office located in Tukwila, WA. You will ensure patient care is coordinated and supervise clinical personnel to deliver quality home care services.

The role requires a WA RN license, with BSN preferred, and prior hospice/home care management experience. Strong observation, judgment, and communication skills are essential to success.

Qualifications

• Valid RN license to practice in Washington state.
• BSN preferred; at least 1 year of nursing experience if no BSN.
• Hospice/home care experience and 2+ years of supervisory experience.
• HCHB and PointCare experience preferred.
• Excellent observation, nursing judgment and communication skills.
• Understands hospice philosophy and pain/symptom management.

Responsibilities

• Coordinate hospice/home care referrals and assign personnel.
• Review cases and guide clinicians to ensure quality home care.
• Oversee medical records management and EMR workflow.
• Attend case conferences with hospice staff to coordinate care.
• Assist in hiring, orientation, and staff development.
• Contribute to policies, procedures and quality improvement efforts.
• Promote customer service orientation and community relations.
• Perform other duties as delegated by Clinical Director.
