Job summary

Affinity Hospice Management LLC is seeking a Clinical Manager to ensure coordinated, high-quality patient care. The role involves supervising clinical staff, reviewing cases, and guiding clinicians to deliver effective home care services.

This position may operate across Snohomish (Lynnwood) and Tukwila locations. The ideal candidate is a licensed RN, with BSN preferred or at least one year of recent nursing experience, and 2+ years of management or supervisory experience in hospice or home

Qualifications

• RN with current state licensure.
• BSN preferred or 1 year nursing experience in last 3 years.
• 2+ years management or supervisory experience in hospice/home care.
• Complies with professional standards and practice.
• HCHB and PointCare experience preferred.
• Excellent observation and communication skills; understands hospice and pain management.

Responsibilities

• Coordinate patient care and supervise clinical personnel.
• Review cases and assign hospice staff; consult Medical Director on eligibility.
• Oversee medical records management in EMR; ensure quality home care services.
• Assist in planning care goals, priorities, and plan of care for patients.
• Participate in case conferences and quality improvement activities.
• Support staff onboarding, orientation, and ongoing education.
