Job summary

Affinity Health Management is seeking a Clinical Manager (RN) to oversee patient care coordination, clinical supervision, and care delivery across Snohomish and Tukwila locations. The role requires active RN licensure, BSN preferred, and 2+ years of management experience in hospice/home care.

You will oversee case reviews, EMR workflows, and staff development while ensuring compliance with professional standards and CPR certification. A valid driver’s license is required.

Qualifications

• Registered nurse licensed to practice in the state.
• BSN degree preferred or 1 year nursing experience in the last 3 years.
• Two years of management or supervisory experience in hospice/home care.
• CPR certification current.
• HCHB and PointCare experience preferred.
• Valid driver’s license and insured vehicle.

Responsibilities

• Coordinate patient care across hospice/home care teams.
• Supervise clinical personnel and reviews case referrals.
• Discuss eligibility with Medical Director as needed.
• Oversee medical records management in EMR system.
• Assist in planning short- and long-term care goals.
