Job summary

Trustbridge Hospice, a member of Empath Health, is seeking a compassionate RN to join the Admissions Team for Broward County. This full-time role supports patient families through one of life’s transitions with empathy and clear communication.

Responsibilities include evaluating referred patients, educating families, coordinating individualized care plans, and serving as a liaison with hospitals and healthcare partners to ensure timely, compassionate hospice care.

Qualifications

• Must hold Florida RN license with current, active status.
• BSN preferred but not required for consideration.
• Hospice certification preferred; BLS required or active.

Responsibilities

• Evaluate patients referred for hospice services.
• Educate patients and families about hospice care and plans of care.
• Coordinate individualized hospice plans of care with physicians and liaisons.
