Job summary

Suncoast Hospice, part of Empath Health in Pinellas County, is seeking an Admissions RN to join our compassionate team delivering exceptional end-of-life care.

You will coordinate patient admissions across long-term care facilities, home, hospital, and hospice settings, educate families, and assess needs to support comfort and dignity.

Qualifications

• Active Florida RN license.
• Minimum one year experience in an acute-care hospital/in-patient setting providing direct patient care; hospice experience preferred.
• BSN or MSN preferred.

Responsibilities

• Coordinates patient admissions activities across long-term care facilities, patient home, hospital, and hospice settings.
• Educates patients, families and caregivers about disease processes, palliative interventions, and safety practices.
• Provides initial assessment of the impact of terminal diagnosis on physical, functional, psychosocial, and environmental needs.

Suncoast Hospice, a part of Empath Health is seeking a caring and motivated Admissions RN to work with us in Pinellas county providing exceptional care on the following

schedule

Sunday- Wednesday 10am -8pm

What You'll Do

• Coordinates patient admissions activities in the long term care facility, patient home, hospital, assisted living facility and hospice house settings.
• Educates patient, family and caregivers regarding disease process and decline, palliative interventions, care giving, dying process, ethical concerns and safety practices.
• Provides initial assessment of the impact of the terminal diagnosis on the patient’s physical, functional, psychosocial and environmental needs and activities of daily living.

What You'll Need

• Must possess a clear and current Florida Licensure as RN. BSN or MSN preferred.
• Minimum one year experience in an acute-care Hospital / In-patient environment providing direct patient care; previous experience as a Hospice RN preferred.
