Job summary

Cynet Health seeks an RN for Home Health intake coordination on a 13-week contract. The role handles referrals, scheduling, and clinical guidance, ensuring smooth coordination among staff and community partners.

Responsibilities include verifying insurance, managing intake data, and supporting a multi-disciplinary team to assure timely patient visits and high-quality home care adherence.

Qualifications

• Associates/Technical Degree in nursing or equivalent combination of education/related experience required.
• Three years' experience in Home Care preferred.

Responsibilities

• Processes home care referrals and coordinates among the clinical staff, referral source, and community liaison.
• Ensures patients are properly entered into the system and scheduled for a visit.
• Provides clinical guidance and identifies educational opportunities.
• Critically assesses patients’ conditions, progress, and ongoing needs.
• Enhances case managers' knowledge of patients' home care needs.
• Receives and appropriately documents new referrals in a timely manner.
• Participates in coordinating care with patients and the interdisciplinary team.
• Interfaces with physician offices and community liaisons to assess and process referrals and admissions for service.
• Provides information and assistance to community members with home and community-based needs.
• Responds to patient and agency needs in a professional and creative manner.
• Distributes referrals to appropriate clinicians that include intake data, essential background information, and plans of care.
• Works independently, meets time deadlines, and manages multiple tasks.
