Job summary

Qualifications

• At least 1.5 years of relevant intake experience.
• Active RN, LVN, or LPN license.
• CPR certification required.
• Epic experience required.
• Associate degree in Nursing or equivalent education/experience required.

Responsibilities

• Process home health referrals and coordinate patient admissions.
• Communicate with physicians and healthcare teams.
• Ensure patients are scheduled for appropriate services.
• Verify insurance benefits and maintain patient records.
• Answer calls and coordinate referrals with clinical staff.
