Job summary

Team Select Home Care is seeking a qualified Case Manager RN to oversee the care of assigned patients in home health settings under physician orders. You will collaborate with physicians, nurses, and therapists to develop patient-centered care plans and ensure accurate documentation.

The role includes conducting initial evaluations, coordinating between families and providers, and mentoring staff while delivering compassionate, high-quality home care. Exemption applies.

Qualifications

• Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing.
• Minimum of 1 year nursing experience; home health experience preferred.

Responsibilities

• Provide and oversee the care of assigned patients according to physician orders and plan of care.
• Develop, implement, and evaluate individualized patient-centered care plans.
• Conduct initial evaluation visits and regular re-evaluations of patient nursing needs.
• Coordinate care between patients, families, and healthcare providers; advocate for patient welfare.
• Prepare clinical and progress notes; participate in case conferences; document required data sets.
• Medication reconciliation, on-call duties, and supervision of ancillary staff as assigned.
• Ensures compliance with policy, infection control, and quality assurance programs.
