Job summary

Team Select Home Care is seeking a qualified RN Case Manager to oversee patient care plans and coordinate services in the Boston area. You will perform initial evaluations, supervise care, and ensure compliant documentation and communication with physicians and families.

The role requires state RN licensure, at least 1 year of nursing experience (home health preferred), and strong organizational and inter-personal skills. On-call responsibilities may apply, with a team-focused environment.

Qualifications

• Graduate of an approved school of professional nursing and currently licensed as an RN in the applicable state.
• Minimum 1 year nursing experience; home health preferred.
• Ability to organize work procedures, supervise ancillary disciplines, and meet state requirements.
• Must maintain health requirements and CPR certification.
• Excellent verbal and written communication skills.

Responsibilities

• Develop and implement individualized patient care plans.
• Provide case management for an assigned group of patients and staff.
• Complete required data collection sets and documentation (OASIS, PAR) timely per regulations.
• Perform initial evaluations and regular reassessments of patient nursing needs.
• Coordinate care with patients, families, and other providers or facilities.
• Administer medications per physician orders and educate caregivers.
