Job summary

Team Select Home Care is seeking a qualified Registered Nurse Case Manager to oversee in-home patient care in the Miami area. You will coordinate care with physicians, therapists, and the patient’s family, ensuring plans of care align with state guidelines and payer requirements.

Responsibilities include developing and updating patient-centered care plans, performing initial evaluations and regular reassessments, documenting progress, and providing education on home health strategies.

Qualifications

• Graduate of an approved nursing program and current RN license in the states practiced.
• Ability to develop and evaluate patient-centered care plans under physician orders.
• Strong communication and coordination with patients, families, and care teams.

Responsibilities

• Develops and oversees individualized care plans for assigned patients.
• Provides case management and coordinates care with physicians, therapists, and staff.
• Documents patient visits, symptoms, and changes per policy and payer requirements.
• Conducts initial evaluations and regular reassessments of patient nursing needs.
• Educates patients and families on home health strategies and resources.
• Manages medications and reconciles changes with physician orders.
• Attends case conferences and reports changes in patient conditions.
