Job summary

Team Select Home Care is seeking a Case Manager RN to oversee patient care plans and coordinate services across multiple providers in the Houston, TX area. The role involves initial assessments, ongoing evaluations, and advocacy for patient welfare, while ensuring adherence to physician orders and state regulations.

You will collaborate with physicians, families, and ancillary staff to deliver comprehensive home health services and ensure timely documentation and care updates.

Qualifications

• Registered nurse authorized to practice per state Act
• Minimum 1 year nursing experience; home health preferred
• Ability to coordinate care across Patients, families, and providers

Responsibilities

• Develop and oversee individualized patient care plans under physician supervision
• Provide case management for assigned patients and staff
• Collect and submit required documentation (OASIS, PAR, etc.) within timelines
• Perform initial evaluation and regular reassessment of nursing needs
• Advise and coordinate care among patients, families, agencies, and other providers
