Job summary
Optimal Home Care in Colorado seeks a Registered Nurse to provide dedicated home health services to patients across the region.
You will perform SOC/ROC/RECERTs, document per Medicare guidelines, supervise LPNs, coordinate care, and collaborate with clinicians and care teams to support quality patient outcomes. Qualifications
- Graduate of an accredited nursing program.
- Active Colorado RN license in good standing.
- Minimum 1 year nursing experience; home health preferred.
- Valid driver’s license.
Responsibilities
- Complete SOC/ROC/RECERTs and supervisory visits for assigned patients.
- Document timely per Medicare guidelines.
- Supervise LPNs and manage nursing visit utilization.
- Coordinate care across accounts/territory and attend resident reviews.
- Communicate consistently with facility staff and care teams.
- Support census growth and collaborate with marketers; review fiscal outcomes.
- Prepare fiscal reports and participate in relevant care coalitions.
- Anticipate 40+ hour work weeks, Monday to Friday as needed.
