Optimal Home Care is seeking a Registered Nurse to provide direct patient care across assigned accounts. You will complete SOC/ROC/RECERTs, supervise LPNs, and manage nursing visit utilization while maintaining thorough documentation and communication with facility staff.

Qualifications

• RN degree from an accredited program and current license in good standing
• Strong communication and documentation skills are essential

• Complete SOC/ROC/RECERTs, visits, and required documentation
• Supervise LPNs and manage nursing visit utilization
• Coordinate care across facilities, participate in reviews, and communicate with staff and marketers
• Generate fiscal reports and support care coordination with leadership
