Optimal Home Care, Inc. is looking for a Registered Nurse to provide direct patient care across our Colorado service area. The role involves calling on accounts, performing SOC/ROC/RECERTs, supervising LPNs, and ensuring high-quality nursing visits within Medicare guidelines.

You will coordinate with facility staff and marke­ting teams to support census growth and patient outcomes. A CO RN license and at least one year of RN experience are required.

Qualifications

• Active Colorado RN license in good standing.
• Graduate of an accredited nursing program.
• At least 1 year RN experience; home health a plus.
• Familiarity with OASIS preferred.
• Must communicate effectively and document client activity.

Responsabilités

• Complete SOC/ROC/RECERTs, supervisory visits, and revisits as needed (10–12 per week).
• Complete documentation per agency and Medicare guidelines on time.
• Provide RN care within scope of practice and supervise LPNs.
• Coordinate care and oversee accounts/territory; attend resident reviews.
• Maintain communication with facility staff and collaborate with marketers.
