Job summary

UnitedHealth Group is seeking a Registered Nurse in Home Health to provide and direct nursing care to patients in their homes, coordinating with healthcare professionals to ensure continuity of care. The role is based in Monroe, LA and offers a full-time schedule.

You will initiate and adjust care plans, supervise home health aides, perform supervisory visits, and conduct comprehensive OASIS assessments with accurate medication reconciliation.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• 1+ years of clinical experience as a Registered Nurse.
• Ability to work independently and meet patient care requirements.

Responsibilities

• Initiate, develop, implement, and revise the plan of care in collaboration with the physician and other healthcare professionals.
• Supervise care provided by home health aides and licensed practical/vocational nurses.
• Provide required supervisory visits.
• Provide high-quality clinical services within the scope of practice and infection control standards.
• Complete comprehensive assessments (OASIS) including medication reconciliation accurately and timely.
• Document patient visits per policy and payer requirements.
• Make initial and/or comprehensive nursing evaluation visits to ensure home health eligibility.
• Communicate relevant information timely and effectively with appropriate agency staff.
• Assist in the orientation of new agency personnel and serve as a preceptor to other staff and students.
• Participate in on-call and weekend rotation as needed to meet patient needs.

Requirements

Education

• Current and unrestricted RN licensure in state of practice

Experience

• 1+ years of clinical experience as a Registered Nurse
• 1+ years of clinical experience may be waived for a Registered Nurse with recent clinical experience as an LPN

Licenses & Certifications

• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
• RN licensure must have no restrictions

Preferred Qualifications

• Current CPR Certification or ability to complete within 90 days of hire
• 1+ years of Home Health experience
• Ability to work independently
• Solid communication, writing, and organizational skills
