Job summary

Fresenius Medical Care in San Antonio, TX is seeking an entry-level Home Therapies RN in the CAP program to coordinate and deliver clinically competent care, with one-on-one patient training on home dialysis and close supervision.

You will assess, educate, and support patients and families, administer medications per protocol, and collaborate with the interdisciplinary team to ensure safe dialysis and CAP progression.

Qualifications

• Entry level for RNs with less than 2 years of Nephrology Nursing experience as a Registered Nurse.
• Minimum of 1-year experience as a Registered Nurse (preferred).
• Home dialysis therapy experience (preferred).

Responsibilities

• Perform nursing duties under supervision with Educator/Preceptor or in collaboration with another Registered Nurse.
• Collect and analyze dialysis data; adjust treatment plan as needed.
• Educate patients and families on home dialysis.
• Provide safe delivery of dialysis therapy at home with care team.
• Administer medications per protocol.
• Initiate or assist with emergency response measures.
• Assist with staff training and onboarding.
• Ensure correct lab collection, processing and shipping.
• Document nursing assessments and plans with Interdisciplinary Team.
• Discuss transplant and treatment modalities with patients.
• Identify home environment barriers for home dialysis.
• Train patients/home partners on equipment operation and maintenance.
• Complete CAP requirements to advance.
• Perform other duties as assigned.
