Qualifications

• Graduate of an accredited School of Nursing.
• Current state RN licensure.
• Current or valid CPR/BLS certification.
• CAP requirements to advance in the program.

Responsibilities

• Educate and coordinate care for home dialysis patients.
• Assess and document patient needs; adjust treatment plans as indicated.
• Train patients and Home Partners on safe operation of home dialysis equipment.
• Collaborate with interdisciplinary team and physicians.
• Maintain accurate nursing documentation and ensure compliance with policies.
