BrightStar Care of Concord, Lexington and Woburn is seeking an experienced Registered Nurse (RN) to lead and deliver high-quality in-home care. You will develop care plans, oversee staff, ensure regulatory compliance, and drive clinical excellence across our home health team.

Qualifications

• Active RN license in the state of operation.
• 2–3 years of home health, hospice, or skilled nursing facility experience.
• Proficient in IV infusion therapy, medications, hydration, and TPN.
• Wound care including dressing changes and NPWT/Wound VAC management.
• Sales and business development mindset to attract and retain clients/referral partners.
• CPR certification is required.
• Care plan development and oversight experience.
• Strong problem-solving and decision-making abilities in staffing and operations.
• Reliable vehicle, valid driver's license, and auto insurance.
• Flexibility and on-call availability for emergencies.

Responsibilities

• Develop and implement patient care plans with physicians and other healthcare providers.
• Conduct in-home assessments ensuring regulatory and company standards.
• Ensure compliance with home health regulations, documentation and reporting.
• Supervise and manage the home care nursing team, including hiring and training.
• Provide guidance to nurses and caregivers to maintain high-quality care.
• Manage scheduling and ensure adequate staff coverage for all cases.
