New Solutions is seeking an RN Home Care Manager in California to lead our home health team and elevate patient care across the community. The role emphasizes clinical supervision, care coordination, staff development, and adherence to regulatory standards to ensure high-quality outcomes.

Qualifications

• RN license to practice in the state.
• 5 years RN experience, with home care background.
• 2 years in management or supervisory role in home health preferred.
• Knowledge of Medicare Conditions of Participation and relevant regulations.

Responsibilities

• Provide clinical supervision to ensure patient care aligns with professional standards, policies, and regulatory requirements.
• Plan, coordinate, and monitor patient care to ensure high-quality outcomes and consistent service delivery.
• Supervise, support, and coach clinical personnel to maintain care excellence and encourage ongoing staff development.
• Facilitate effective, transparent communication among patients, families, referral sources, and care team members.
• Drive quality improvement initiatives and meet departmental performance goals.
• Model professional leadership, service excellence, and core organizational values.
• Maintain rigorous compliance through required training, accurate documentation, and audit cooperation.

Position Responsibilities:

• Provide clinical supervision to ensure patient care strictly aligns with professional standards, policies, and regulatory requirements.
• Plan, coordinate, and monitor patient care to ensure high-quality outcomes and consistent service delivery.
• Supervise, support, and coach clinical personnel to maintain care excellence and encourage ongoing staff development.
• Facilitate effective, transparent communication among patients, families, referral sources, and care team members.

• Registered nurse with current licensure to practice nursing in the state.
• 5 yrs RN experience with previous experience in home care setting.
• 2 years management or supervisory experience in home health, preferred.
• Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirements
