RN Home Health in Arlington, TX is seeking a skilled Registered Nurse for field-based home health case management. You will manage a patient caseload, perform nursing visits, and develop care plans in collaboration with physicians and families.

Ideal candidates have strong assessment and documentation skills, plus a solid understanding of Medicare home health and OASIS. Excellent communication with the clinical team is essential for success.

Qualifications

• Active RN license in good standing.
• Recent Home Health RN case management experience required.
• Strong assessment and documentation skills.
• Ability to manage an assigned patient caseload independently in the field.
• Solid understanding of Medicare home health, OASIS, plan-of-care management.

Responsibilities

• Manage an assigned home health patient caseload in the field.
• Perform skilled nursing visits, assessments, evaluations, recertifications, resumptions of care, and discharges.
• Develop, update, and coordinate individualized plans of care in collaboration with physicians, patients, families, and the interdisciplinary care team.
• Complete accurate, timely clinical documentation, including OASIS and visit notes.
• Monitor patient progress, identify changes in condition, and communicate updates appropriately.
• Provide patient and caregiver education related to disease management, medications, safety, and plan-of-care goals.

This position is best suited for a clinically strong Registered Nurse with recent home health case management experience, strong assessment skills, solid documentation habits, and the ability to manage an assigned patient caseload independently in the field.

Position Responsibilities:

• Manage an assigned home health patient caseload in the field.
