KLA Industries is seeking a Patient Care Manager (RN) to lead our home health team and elevate patient care across the community. You will empower clinical staff, ensure regulatory compliance, and foster a culture of safety, quality, and compassionate service.

As a key clinical leader in California, you will guide staff development, drive process improvements, and collaborate with families and referral sources to deliver consistent, high-quality home health care.

Qualificações

• Current RN license in the state of practice.
• Minimum 5 years of RN experience, preferably in home health.
• 2 years of supervisory or management experience in home health preferred.
• Knowledge of Medicare Conditions of Participation and applicable regulations.

Responsibilities:

• Provide clinical supervision to ensure patient care strictly aligns with professional standards, agency policies, and regulatory requirements.

• Plan, coordinate, and monitor patient care to ensure high-quality outcomes and consistent service delivery.

• Supervise, support, and coach clinical personnel to maintain care excellence and encourage ongoing staff development.

• Facilitate effective, transparent communication among patients, families, referral sources, and care team members.

• Drive quality improvement initiatives and meet departmental performance goals.

• Maintain rigorous compliance through required training, accurate documentation, and audit cooperation.

Qualifications:

• Registered nurse with current licensure to practice nursing in the practicing state.
• 5 yrs RN experience with previous experience in home care setting.
• 2 years management or supervisory experience in home health, preferred.
• Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirements
