New Solutions is seeking an RN Home Care Manager to lead our home health team and elevate patient care across the community. You will empower clinical staff to foster a collaborative, supportive environment where care teams thrive and deliver compassionate home health services.

Responsibilities include supervising staff to ensure care aligns with standards, planning and monitoring patient care, and driving quality improvements while maintaining strict regulatory compliance.

Qualifications

• Minimum 5 years RN experience, with prior home health background.
• 2 years in management or supervisory roles in home health preferred.
• Knowledge of professional standards and Medicare Conditions of Participation, plus regulatory requirements.

Responsibilities

• Provide clinical supervision to ensure patient care aligns with standards and regulations.
• Plan, coordinate, and monitor patient care for high-quality outcomes.
• Supervise, support, and coach clinical staff to maintain care excellence and staff development.

As an RN Home Care Manager, you play a pivotal role in leading our home health team and elevating patient care across the community.

In this role, you will empower and guide clinical staff to foster a collaborative, supportive environment where care teams thrive and deliver exceptional, compassionate home health services.

Position Responsibilities:

• Provide clinical supervision to ensure patient care strictly aligns with professional standards, policies, and regulatory requirements.
• Plan, coordinate, and monitor patient care to ensure high-quality outcomes and consistent service delivery.
• Supervise, support, and coach clinical personnel to maintain care excellence and encourage ongoing staff development.
• Facilitate effective, transparent communication among patients, families, referral sources, and care team members.
• Drive quality improvement initiatives and meet departmental performance goals.
• Model professional leadership, service excellence, and core organizational values.
• Maintain rigorous compliance through required training, accurate documentation, and audit cooperation.

• Registered nurse with current licensure to practice nursing in the state.
• 5 yrs RN experience with previous experience in home care setting.
• 2 years management or supervisory experience in home health, preferred.
• Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirements
