Job summary

Nationwide Healthcare Services seeks a Home Care Coordination RN to supervise staff and ensure regulatory compliance for home care and PACE center activities. The role centers on coordinating the plan of care, scheduling, communication, and documentation of all care aspects.

You will conduct enrollment assessments, participate in on-call coverage, supervise caregivers, and maintain quality records within the Interdisciplinary Team framework.

Qualifications

• Active RN license and registration in good standing.
• Experience coordinating home health care services.
• Familiarity with care plans, assessments, and EMR documentation.

Responsibilities

• Manage staff roster and schedules to meet home care needs.
• Design training and supervision systems for caregivers.
• Supervise staff and evaluate performance.
• Communicate with participants and families about home care needs.
• Maintain accurate home care records and billing documentation.
• Coordinate equipment, supplies, and services per care plan.
• Participate in enrollment assessment and care plan development.
• Collaborate with the Interdisciplinary Team and hospital departments.
