Job summary

Northwestern Medicine is seeking a Home Care Coordinator to align hospital-based care with post-discharge home health services. You will collaborate with physicians, nurses, social workers and discharge planners to build a seamless care plan for patients transitioning from Palos Health to home care.

Responsibilities include attending rounds, coordinating services, educating families, and documenting all communications while ensuring HIPAA compliance and adherence to agency policies.

Qualifications

• Current licensure as a Registered Nurse in Illinois.
• Minimum 2 years of previous home health experience.
• BSN preferred.

Responsibilities

• Attends interdisciplinary rounds to facilitate coordination of services from hospital to home.
• Consults with physicians, nurses, social workers and the health care team to determine patient needs and plan care prior to discharge.
• Facilitates discharge planning for Home Care patients from hospital.
• Identifies hospitalized patients who may require initiation of home care services.
• Educates patients and families regarding available resources post hospitalization.
• Documents all communication with care team and discharge planners.
• Generates monthly tracking reports as requested.
