Primary responsibilities include assessing, planning, and facilitating safe, efficient discharges while supporting the care team and ensuring quality outcomes in a patient-centered environment.

Qualifications

• ADN is required.
• BSN is preferred.
• Two years of nursing experience required.
• Excellent communication and organizational skills.

Responsabilités

• Coordinate care using the nursing process with the care team.
• Engage patients and caregivers to assess self-care and post-hospital needs.
• Coordinate care across the continuum to achieve quality and cost outcomes.
• Participate in daily multi-disciplinary rounds to identify discharge needs.
• Facilitate communication among the healthcare team, patients, and external partners.
• Assist other departments with care coordination and discharge planning.
• Perform admission, concurrent and discharge reviews to ensure appropriate utilization.
