Job summary

Rochester Regional Health is seeking a Home Care Coordinator- Registered Nurse to facilitate patient discharge from facility settings to certified home care services at the patient’s residence.

The role emphasizes delivering superior customer service in line with the Brand Promise and Core Values, coordinating in-home services, and completing essential documentation for care plans and reporting.

Qualifications

• AAS in Nursing required.
• Valid NYS RN license required.
• BSN is preferred for this role.

Responsibilities

• Evaluates eligibility, needs and care resources for each patient referred to RRH Home Care.
• Completes a comprehensive clinical assessment for home care services and develops a home care plan with the patient/caregivers.
• Obtains MD orders to support the home care plan.
• Coordinates all in-home services as dictated by the care plan.
• Completes required documentation in patient records and RRH Home Care documentation.
• Gathers and analyzes data and completes assigned reports.
• Collaborates with facility care coordinators, social workers, and physicians to resolve complex discharge needs.
• Participates in RRH Home Care committees or groups.
• Represents RRH Home Care in community groups and health care settings.
• Participates in marketing efforts and serves as a community resource education focus.
