Job summary

Northwestern Memorial HealthCare seeks an Home Care Coordinator to bridge hospital and home-based services for Palos Health patients. The role requires an IL-licensed RN with at least 2 years in home health, and involves discharge planning, care coordination, and patient education.

You will collaborate with physicians, nurses, and social services to tailor post-discharge plans, ensure regulatory compliance, and document all communications and outcomes.

Qualifications

• Current licensure as a Registered Nurse in Illinois.
• Minimum 2 years of home health experience.
• BSN preferred (optional).

Responsibilities

• Attends interdisciplinary rounds to coordinate services from hospital to home.
• Consults with physicians, nurses and social services to determine patient needs.
• Facilitates discharge planning for Home Health patients and inpatients.
• Identifies patients who may need initiation of services after hours.
• Educates patients and families on available post-discharge resources.
• Documents all communications with care team members and families.
• Generates monthly tracking reports and assists with benefits verification.
