Qualificações

• Associates of Nursing required.
• 2+ medical/hospital nursing experience required.
• RN license required (RN) and BLS preferred.
• Knowledge of discharge planning processes and community resources is essential.

Responsabilidades

• Participates in multidisciplinary rounds to review patient status and discharge plans.
• Communicates with patients/families about social, emotional, and financial aspects of illness.
• Assesses readmission risks and develops discharge planning strategies.
• Documents discharge plans, barriers, and progression in medical records.
• Coordinates post-acute care authorization and referrals as needed.
