Job summary

Signature HealthCARE is seeking an experienced Registered Nurse for full-time night/day shifts at our Bowling Green facility. You will deliver direct nursing care, supervise aides, and coordinate care plans to ensure quality resident outcomes.

Ideal candidates hold an active RN license in Kentucky, 1+ year of clinical experience (long-term care preferred), and familiarity with EHR systems. We offer a comprehensive benefits package and a sign-on bonus.

Qualifications

Current, active RN license in the state of practice.
CPR/BLS certification preferred (American Heart Association or American Red Cross).
Minimum one year of clinical nursing experience; long-term care experience preferred.
Experience in a supervisory or charge nurse role, preferred.
Familiarity with electronic health records (EHR) systems.
Effective verbal and written communication skills.

Responsibilities

Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.
Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
Complete accurate and timely clinical documentation per facility policy and procedure.
Coordinate resident admissions, transfers, and discharges according to established procedures.
Participate in quality assurance activities, staff education, and regulatory surveys as needed.
Perform other duties as assigned by the Director of Nursing or designee.
