Signature HealthCARE Heritage Hall Rehab & Wellness Center in Kentucky is seeking an RN to deliver direct nursing care, supervise nursing assistants, and manage care plans for residents. You will document care, monitor health status, and coordinate admissions and discharges in a team-oriented, resident-centered environment.

Requirements include an active RN license in the state, 1 year of clinical nursing experience (long-term care preferred), and familiarity with EHR systems; CPR/BLS is a plus.

Qualifications

• Current, active RN license in the state of practice.
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.
