Job summary

Signature HealthCARE's North Hardin Rehab & Wellness Center in Radcliff, KY, is seeking a dedicated RN to join our team delivering high-quality rehabilitation and long-term care. This position offers night shifts (7 PM–7 AM) and opportunities to lead and mentor staff within an interdisciplinary team.

We require an active RN license and strong clinical/documentation skills; supervisory experience is welcome. A $10,000 sign-on bonus, comprehensive benefits, and growth opportunities are available.

Qualifications

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Knowledge of EHR systems and Microsoft Office.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs and ensure comfort, dignity, and safety.
• Supervise nursing assistants and medication aides during shifts.
• Monitor residents' status and notify physician of changes.
• Develop and review individualized care plans with the interdisciplinary team.
• Complete accurate clinical documentation per facility policy.
• Coordinate admissions, transfers, and discharges according to procedures.
• Participate in quality assurance activities and staff education.
• Perform other duties as assigned by the Director of Nursing.
