Job summary

Providence PACE is seeking a Flex RN to provide case management, home health visits, and clinic nursing within South King County. The role supports the PACE RN Case Manager and clinic/home health nurses, utilizing advanced nursing skills for care coordination and triage.

You will be part of an interdisciplinary team delivering wrap-around care to frail elders, with a Monday–Friday day shift and no weekends or holidays.

Qualifications

• Education sufficient for clinical licensure.
• Upon hire: Washington Registered Nurse License.
• Upon hire: National Provider BLS - American Heart Association.
• 1 year Long term care or geriatric nursing.

Responsibilities

• Develop an integrated plan of care and provide direct nursing care to frail elderly participants.
• Support and provide coverage for PACE RN Case Manager, clinic nurse and home health nurse positions.
• Utilize high level nursing skills from care coordination and triage to hands-on clinical nursing tasks.
• Deliver wrap-around care as part of an interdisciplinary team.

$10,000 hiring bonus for eligible external candidates that meet required qualifications and conditions for payment.

This is a flex nursing role which provides case management, home health and clinic visits in South King county.

Schedule: Weekdays Monday to Friday Day shift. No weekends or holidays!

As a member of the Interdisciplinary Team, the Registered Nurse (RN) participates in the development of an integrated plan of care and provides direct nursing care to our frail elderly participants. The Flex RN position supports and provides coverage for the PACE RN Case Manager position and PACE clinic nurse and home health nurse positions. This dynamic position utilizes the full-spectrum of high level nursing skills-- from critical -thinking care coordination and triage skills, to hands-on, clinical nursing skills.
