Qualifications

• WA RN License required at hire.
• 1 year Long term care or geriatric nursing experience.

Responsibilities

• Provide direct nursing care to frail elderly participants.
• Develop and implement an integrated plan of care with the Interdisciplinary Team.
• Support PACE RN Case Manager position and clinic/home health nursing as needed.

Washington Registered Nurse License
