Job summary

Providence is seeking a Social Worker to join our interdisciplinary team, delivering direct social work services to participants within the Center. You will perform assessments and develop integrated care plans addressing social, emotional, cognitive, and ethnic factors while linking participants to community resources for continuity of care.

The role emphasizes geriatric care, with at least two years of geriatric social work and one year of experience with older adults.

Qualifications

• Master's Degree in Social Work required.
• Washington Clinical Independent Social Worker Associate License or temporary license or full license required on hire.
• 2 years Social Work with geriatric age groups from diverse ethnic cultures.
• 1 year work experience with geriatric population.

Responsibilities

• Provide social work services to participants in direct service activities within the Center.
• Conduct social work assessments and intervenions; address social, emotional, cognitive and ethnic factors for care plans.
• Access community and financial resources for participants and ensure continuity of care across the continuum.
• Perform social work assessment and intervention in other settings, including the participant's home, as needed.

Schedule : Monday- Friday Day shift

As a member of the interdisciplinary team, provides social work services to participants in a manner which is consistent with the mission and core values of Providence Health System. Spends a majority of time in direct service activities within the Center, including one-on-one and group counseling, interaction with other team members, and ongoing assistance to participants to address social issues. Based on comprehensive assessments, provides perspective on the social, emotional, cognitive and ethnic factors that impact the creation of an individualized integrated plan of care. Accesses community and financial resources for participants and ensures continuity of care across the continuum. Conducts social work assessment and intervention in other care settings, including the participant's home, as needed.
