CorVel Corporation is seeking a licensed registered nurse for a Field (Medical) Case Manager role in the Fairfax, VA area. The position involves both home-based and on-site travel with a heavy local travel component and regular daytime hours.

You will collaborate with patients, families, and providers to facilitate recovery, assess treatment plans, and support coordinated care. Travel requirements and CCM certification preferences are noted in the qualifications.

Qualificações

• Current RN licensure in the state of operation.
• Graduate of an accredited school of nursing.
• CCM, CIRS, or other case management certifications preferred.
• Ability to travel to assigned locations (local/overnight).
• Experience as an RN Medical Case Manager ideal.

Responsabilidades

• Provide in-person and telephonic Medical Case Management to individuals, involving patient, physician, other health care providers, employer, and referral source.
• Collaborate with patients, families, medical providers, and team members to facilitate recovery.
• Travel locally up to ~60% of the work schedule; overnight travel may occur.
• Perform assessment, planning, implementation, and evaluation of patient progress and outcomes.
• Discuss treatment plans with physicians and discuss alternatives as needed.
