Job summary

Qualifications

• Must have active RN license in state of practice.

Responsibilities

• Complete comprehensive member assessments within regulatory timelines.
• Develop and implement care coordination plans with the care team.
• Conduct telephonic, in-person, or home visits as required.
• Monitor care plans and document interventions and outcomes.
• Maintain ongoing member caseload and outreach.
• Facilitate interdisciplinary care team meetings and collaboration.
• Coordinate with behavioral health, LTSS, and community resources.
• Provide consultation to peers as needed.
