Job summary

Humana seeks an experienced RN Field Care Manager for Maternal-Child Health in Louisiana to assess needs and guide families toward wellness. You will develop care plans and coordinate with PCPs and community resources, addressing physical, social, and environmental determinants.

Travel up to 50% within Louisiana; remote/home office when not in visits. Requires active LA RN license and 2+ years in MCH/L&D/pediatrics; strong communication and independent judgment are essential.

Qualifications

• Active RN license in Louisiana without restrictions.
• 2+ years of maternal-child health, labor & delivery, or pediatrics.
• Intermediate to advanced computer skills (Word, Outlook, Excel).

Responsibilities

• Identify and resolve barriers to care.
• Conduct telephonic and in-person assessments.
• Develop individualized care plans using clinical judgment.
• Coordinate services for physical health and social determinants.
• Collaborate with providers and community services.
• Support care transitions and documentation in EMR.
