Qualifikationen

• Active RN licensure in Louisiana.
• Minimum 2 years maternal-child health, Labor & Delivery, or pediatrics.
• Proficiency with Microsoft Word, Outlook and Excel.
• Experience using EMR and other information systems.

Aufgaben

• Assess and evaluate members' needs to guide them to appropriate resources.
• Develop individualized care plans using clinical judgment.
• Coordinate services with PCPs and care transitions.
• Support health promotion and community events.
• Submit incident reports as required.
• Travel up to 50% within Louisiana for member visits.
