Job summary

Humana in Louisiana is seeking an RN Field Care Manager Nurse 2 (Maternal-Child Health) to assess members’ needs, guide families to appropriate resources, and coordinate with PCPs and community services. You will develop individualized care plans using clinical judgment and monitor progress in a field-based role.

The position requires an active Louisiana RN license, 2+ years in maternal-child health, L&D, or pediatrics, and comfort with EMR systems.

Qualifications

• Active RN license in Louisiana with no restrictions.
• Two or more years of maternal-child health, L&D, or pediatrics experience.
• Proficiency with Word, Outlook and Excel.
• Ability to use electronic medical records and various information systems.

Responsibilities

• Assess members' needs and guide to resources to achieve wellness.
• Develop individualized care plans using clinical judgment.
• Collaborate with PCPs and community services for outcomes.
• Coordinate services across physical health and social determinants.
