Job summary

The role requires an active Louisiana RN license with 2+ years in maternal-child health, L&D, or pediatrics, along with strong communication and EMR skills.

Qualifications

• Active licensed RN in Louisiana without restrictions.
• 2+ years of clinical experience in maternal-child health, L&D and/or pediatrics.
• Proficient with Word, Outlook and Excel.
• Ability to use EMR and other electronic information applications.
• Strong communication and interpersonal skills.
• Ability to work independently with minimal supervision.

Responsibilities

• Assess and evaluate members' needs to achieve optimal wellness.
• Develop individualized care plans using clinical judgment.
• Collaborate with providers and community services to ensure quality outcomes.
• Coordinate services for physical health and social determinants of health.
• Communicate across care teams including PCP and care transitions.
• Provide resources to families for health and wellness support.
• Submit incident reports as required.
• Participate in health promotion and community events.
