Job summary

Qualifications

• Active RN license in Louisiana without restrictions.
• 2+ years clinical experience in maternal-child health, L&D or pediatrics.
• Computer skills: Word, Outlook, Excel; experience with electronic health records.

Responsibilities

• Assess and evaluate members' needs to guide toward appropriate resources.
• Develop individualized care plans using clinical judgment and changes in health status.
• Coordinate services with providers and community resources to ensure quality outcomes.
• Communicate effectively with members, families, and the care team; travel up to 50% within Louisiana.
