Humana is seeking a Field Care Manager Nurse 2 (Maternal-Child Health) to assess and guide members toward care resources. The role includes telephonic and in-person assessments, care planning, and collaboration with PCPs and community services.

Responsibilities cover maternal-child health, L&D, and pediatrics with a strong emphasis on independent decision-making and effective communication. Travel up to 50% across Louisiana regions is required.

Qualificações

• Active RN license in Louisiana with no restrictions.
• 2+ years in maternal-child health, L&D, or pediatrics.
• Proficient with MS Word, Outlook and Excel; comfortable with EMR systems.
• Strong communication and interpersonal skills with the ability to build rapport.
• Ability to work independently and adapt care plans to changing member needs.
