Qualifications

• Active RN license in Louisiana without restrictions.
• 2+ years clinical experience in maternal-child health, Labor & Delivery, or pediatrics.
• Proficient in Microsoft Word, Outlook, and Excel.
• Strong communication and interpersonal skills.
• Ability to work independently with sound judgment and adapt care plans.

Responsibilities

• Assess and guide members toward appropriate resources.
• Develop individualized care plans using clinical judgment.
• Coordinate services across physical health and social determinants of health.
• Collaborate with providers and community services to ensure outcomes.
• Conduct telephonic and in-person assessments as needed.
