Job summary

Humana is seeking a Field Care Manager Nurse 2 (Maternal-Child Health) to assess member needs and guide families to appropriate resources. You will develop individualized care plans, coordinate with PCPs and community services, and monitor progress to improve outcomes.

This role is primarily field-based with remote home-office options when not in the field. The ideal candidate has an active Louisiana RN license, 2+ years in maternal-child health or pediatrics, plus strong communication and EMR

Qualifications

• Active RN license in Louisiana without restrictions.
• 2+ years of clinical experience in maternal-child health, L&D or pediatrics.
• Proficient with EMR and Microsoft Office tools.
• Strong communication and interpersonal skills with ability to build rapport quickly.

Responsibilities

• Identify and resolve barriers hindering effective care.
• Develop individualized care plans using clinical judgment.
• Coordinate services with providers and community resources.
• Conduct telephonic and in-person assessments to identify member needs.
• Support health and wellbeing through regular monitoring and evaluation.
