Job summary

Optum is seeking a PRN Field Assessor in Milwaukee, WI to perform Long Term Care insurance assessments as part of our field team. Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license and access to a dependable vehicle or public transportation.
• Current CPR certification.
• Ability to function in home settings regardless of client demographics.

Responsibilities

• Perform Long Term Care insurance assessments as part of RN Field Assessor duties.
• Obtain claimant demographics, physician information, and current medication list.
• Evaluate claimant's functional level of independence (ADL/IADL).
• Complete a brief cognitive screening and document results in the web-based system.
