Job summary

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver’s license and access to a dependable vehicle or public transportation.
• Current CPR certification.
• Ability to work in home settings with diverse clients.

Responsibilities

• Complete Long Term Care insurance assessments and gather claimant demographics, physician information, and current medication lists.
• Evaluate the claimant’s functional level of independence and perform brief cognitive screening.
• Utilize a point of care web application/system on a laptop or tablet for real-time documentation.
