Job summary
Research Medical Center in Kansas City, MO is seeking a Case Manager to support patients and families through every step of their care journey. You will coordinate services, connect resources, and develop care plans aligned with each patient’s needs.
You will partner with physicians, nurses, and leaders to ensure safe transitions, clear communication, and quality across the continuum of care.
Qualifications
- RN with active license and diploma.
- Experience coordinating patient care across care teams.
- Ability to develop care plans reflecting patient needs.
Responsibilities
- Guide patients and families through program orientation, explain rehabilitation philosophy, benefits, discharge criteria, and patient rights.
- Coordinate education for patients, families, and caregivers to support treatment planning and family conferences.
- Develop and implement individualized treatment plans reflecting patient strengths and needs.
- Complete psychosocial assessments and discharge planning evaluations; document findings and communicate needs across the team.
- Collaborate with Rehab Program Director, Facility Case Management Director, and the interdisciplinary team for seamless patient-centered care.
- Promote HCA Healthcare values of compassion, respect, and excellence in every interaction.
