Job summary

Menorah Medical Center, a full-service acute care hospital serving Overland Park and Leawood, Kansas, is seeking an RN Case Manager to guide patients and families through the care journey.

In this role you will coordinate services, develop individualized care plans, conduct psychosocial assessments, and work with physicians and the rehab team to ensure safe transitions and consistent quality across the continuum of care.

Qualifications

• RN license required.
• RN Diploma or Associate Degree required.
• No travel required.
• Minimum 3 years of experience.

Responsibilities

• Guide patients and families through orientation, rehab philosophy, Medicare and insurance benefits.
• Coordinate education for patients, families, and caregivers to participate in treatment planning and family conferences.
• Develop and implement individualized treatment plans reflecting patient strengths, needs and goals.
• Complete psychosocial assessments and discharge planning evaluations, documenting findings and communicating needs to the care team.
• Collaborate with Rehab Program Director, Facility Case Management Director, and the interdisciplinary team for seamless, patient-centered care.
• Promote HCA Healthcare values of compassion, respect, and excellence in every interaction.
