Job summary

Qualifications

• RN licensed and with an Associate Degree.
• Experience in case management preferred.
• Ability to coordinate care across multidisciplinary teams.

Responsibilities

• Guide patients and families through orientation, discharge criteria, and patient rights.
• Coordinate education for patients and families to support participation in treatment plans.
• Develop and implement individualized treatment plans reflecting patient goals.
• Perform psychosocial assessments and discharge planning evaluations, documenting findings clearly.
• Collaborate with the Rehab Program Director and interdisciplinary team to ensure seamless care.
• Promote HCA Healthcare values through every patient and family interaction.
