Job summary

Option Care Health is seeking a Clinical Transition Specialist to sell home infusion services to discharging patients, educate families, and coordinate care at alternate sites. The role requires a licensed RN or Pharmacist in the applicable state and at least two years in healthcare, plus proficiency with MS Office.

Occasional travel to care facilities is expected as part of discharge planning. You will partner with intake, discharge planners, and Account Managers to expand referrals and achieve

Qualifications

• Licensed RN or Pharmacist with current state licensure.
• Experience establishing relationships with referrals and healthcare partners.
• Proficient with MS Office suite (Word, Excel, PowerPoint).

Responsibilities

• Develop and maintain relationships with referrals to grow patient starts.
• Educate patients and facility staff about home infusion services.
• Assess patient suitability for alternative-site care and coordinate admissions.
• Collaborate with intake and discharge planners to ensure smooth transitions.
• Partner with Account Manager to execute area growth plans.
• Document communications and ensure confidentiality and compliance.
